I hereby apply for membership of Fitler Club. If accepted, I agree to be bound by the rules of the club.
Please note that all sections must be completed in order for this application to be submitted to the membership committee. Any application missing requested information will be deemed incomplete. All information will be strictly confidential.
My signature below affirms that I understand and agree that: If I become a member of Fitler Club (the "Club"), the initiation payment that I make is not refundable if I decide not to remain a member of the Club.

Membership Process
*A one-time initiation fee will be required upon notification of acceptance.
By completing Fitler Club’s (the “Club”) application for membership, you are authorizing the Club’s membership committee to verify the information in your application. The verification process may include, but is not limited to, interviewing you, contacting your references, performing a background check and obtaining a credit verification report for you. The committee also may request additional information from you as part of its consideration of your application. The committee will send you a written notice of whether your membership has been approved. The committee may approve or reject your membership in its sole discretion, subject to the requirements of law. Continued membership in the Club is predicated on you and your guests complying with Club rules and paying Club dues and fees.
The Club does not discriminate on the basis of race, color, religion (creed), gender, gender expression, age, national origin (ancestry), disability, marital status, sexual orientation, or military status, in any of its activities or operations. These activities include, but are not limited to, the admission of members, hiring and firing of staff, selection of vendors and provision of services.
